- Rules for contract work signed by owners and contractor
- $1000 deposit (check made to the Palm Beach House)
- Copy of worker's Comp insurance
- Copy of Liability insurance with Association listed as additional insured
- Copy of permit application from city (filled out)
- Copy of contractors license
- Spec sheet for type sound proof material being used
- Initial bottom right of each page
